Primary Care - LPN/ LVN

Overview

We’re seeking an experienced Licensed Practical Nurse (LPN) for a Primary Care Float position in Wellsville, NY. This role will support multiple outpatient practices across the network, assisting providers with patient care and ensuring smooth clinical operations in a fast-paced environment.

Responsibilities

1. Room patients and obtain vital signs and medical histories
2. Assist providers with exams and minor procedures
3. Administer medications and immunizations per provider orders
4. Perform basic clinical tasks (EKGs, specimen collection, point-of-care testing)
5. Document accurately in the EMR
6. Support care coordination and maintain exam room readiness

Required Qualifications

1. Active New York State LPN license (required)
2. Current BLS (AHA) certification
3. Minimum 1 year of outpatient or primary care experience
4. Ability to float between multiple clinic locations
5. Must have EPIC experience
6. Must live over 50 miles from Wellsville, NY

Shift / Schedule

1. Day shift
2. Monday–Friday
3. Hours will vary by location, scheduled between 7:00 AM – 6:00 PM
4. No call
5. No weekends (unless otherwise specified by department need)
